The Patient Protection and Affordable Care Act (PPACA)

The Patient Protection and Affordable Care Act (PPACA) is one of the most substantial reforms in Medicare since 1965. This is now considered the law of the land according to Douglas Holtz-Eaton. The PPACA portrays a â€Å"coverage first† strategy. â€Å"Sadly, a review a of the state’s experience bodes poorly for the future of national reform.† (Point/Counterpoint 177) There are two major driving factors in which could propose a threat for this reform. The first factor is it costs too much. Many decades ago, healthcare spending was at a minimum and not the focal point of American citizens. The statics show during 1970, national health expenditures were $1,300 per person and consumed 7 cents out of every national dollar, 7% of the GDP.†¦show more content†¦Dr. Donald Berwick also has an organization in support of Healthcare improvement. According to the Globe, â€Å"every year, up to 98,000 people are believed to die in American hospitals because of medical mistakes†. Berwick is aware of these results and is determined to be a voice in response to those fallen victims. He has come up with a plan that consists of taking the same energy and focus that has transferred other industries, into the healthcare system. He and his institution, along with hundreds of other healthcare organizations, are hard at work in supporting his the ories. Berwick is looked upon as a revolutionary because he is standing up to the healthcare leaders and truly believes and supports that the healthcare system is broken. Dr. Donald Berwick states â€Å"first the healthcare system needs to be blown up†. His vision of a healthcare system would consist of calling the doctor in the morning and receiving an appointment in the afternoon. This appointment would also start on time not three hours later. â€Å"Medication errors-overdoses, allergic reactions, and other adverse responses-would be all but eliminated by the universal adoption of computerized drug-ordering systems.† (Globe 1) Hospitals would implement a â€Å"zero-tolerance† for workers who didn’t wash their hands, which could save 10,000 lives a year. Today we’re putting Joseph Campbell (1904-1987) on the masthead. Chances are that you already know all about his thought and work without realizing it. When George Lucas wrote the first few drafts of Star Wars, it was shaping up to be standard, 70’s sci-fi action schlock. Then he put the screenplay aside to settle and re-read Campbell’s The Hero with a Thousand Faces. That changed everything. Getting started Overview of Rational Performance Tester IBM Rational Performance Tester, hereafter known as Performance Tester, is a performance test creation, execution, and analysis tool that helps development teams validate the scalability and reliability of their Web-based applications before deployment. Many of Performance Tester’s features have been explicitly designed with the novice load tester in mind. Performance Tester allows you to use one of several Web browsers (Internet Explorer, Mozilla, or Firefox) to test a Web-based application. The results of your interaction are captured and recorded on the operating system of your choice (Windows or Linux). The test is presented in a concise tree-based editor that is capable of exposing underlying details to the expert on an â€Å"as needed† basis. Test scripts are then grouped together in various combinations to reflect the multiple types of user that comprise the projected user population. You can specify the number of simulated system users at execution time. Test execution is accompanied by easy-to-read, real-time reports that update throughout the test run. Bottlenecks based on metrics such as round-trip performance, transaction rates, and system diagnostics are highlighted in these reports. You can also use Performance Tester to further identify the root cause of poor performance problems from the hardware- or software-component level through advanced resource monitoring and response-time tracking. Although this tutorial focuses on testing a J2EE, Web-based application, you can use Performance Tester to test any Web-based application. You can also extend Performance Tester to test the performance of additional application types such as Siebel, SAP, and Citrix (see Resources). Once you have registered for the online trial system, installed the Citrix Metaframe Presentation Server, and logged into the server, you are ready to begin. Performance Tester will launch in a Citrix client session and will appear just as if it were running on your local desktop. You can interact with it just as you would if it were installed locally. The countdown clock on the Rational Test Drive Environment Web page will keep track of the time remaining in your session. If you choose to set up the tutorial environment locally you will need to install and configure Rational Performance Tester and WebSphere Application Server 6. 0 in your workstation. Note that the Adventure Builder sample application used by this tutorial is only available with WebSphere Application Server 6. , not 6. 1. You should also allow additional time to create and configure a project in which to store your test artifacts before starting. The tutorial is written from the assumption that you are using the online trial system. Performance Tester and Eclipse Performance Tester is based on the open-source Eclipse platform. Eclipse provides a rich working environment for many tools, both open source and commercial, including many of the offerings of the Rational Software Delivery Platform such as Rational Software Architect, Rational Application Developer for WebSphere Software, and Rational Functional Tester (see Resources). This provides a common user experience for tooling across the software development life cycle. Not only are these tools all based on Eclipse, but often times they actually share the same shell. In other words, the capability of each of these tools is presented to the user as a new perspective in the same shell. A perspective is a consolidation of tools and views focused on one particular task. The perspective for Performance Tester is known as the Test perspective. As the name implies, the Test perspective provides views that are needed by a developer or QA professional focused on testing a software application or system. The tutorial workspace and sample projects Assuming you are using the online trial system, Performance Tester will be associated to a pre-configured workspace. A workspace can be any directory location where your work is stored. In your case, this workspace contains two projects. The first project is the Adventure Builder Application. This project contains the source code for the Adventure Builder sample application you will be testing. The application has already been deployed to WebSphere Application Server on the online trial system. If you expand the project, you will see several folders used to organize your test assets. You can add, remove, and customize folders as you like. There isn’t much to examine in this project yet. Record a test scenario Performance tests are most often created by recording your manual interactions with the system under test. In this section of the tutorial, you will use the automated HTTP recorder to capture the scenario of interacting with the Adventure Builder sample application to construct a vacation package and purchase it. The Adventure Builder application is already running in the online trial environment. All you need to do is connect to it through a Web browser while the recorder is engaged. Browser support Although you are using Internet Explorer in this tutorial, Performance Tester can work with any Web browser that supports SOCKS proxies. Review and customize the test Performance Tester generates a test based on the HTTP traffic it captured during the recording. The test is much more than a simple HTTP trace log, however. Behind the scenes, Performance Tester does a lot of processing to create a test that is robust, extensible, and easy to maintain. When you play-back this test, it will place another order and you will be given a different order ID. You would want Performance Tester to check the status of that new order ID, not the one you previously recorded. For that reason, Performance Tester performs automatic data correlation. That is, it looks at data parameters sent to the server and matches them up with prior response data from the server. Accessing the correlated data is easy. Modern Web applications have many layers of caching. If you were to emulate a thousand users using the application doing exactly the same thing, you would not observe typical performance behavior. Once the first emulated user performed the transaction, all the subsequent users would be drawing information from the cache. For this reason, performance test engineers often spend much of their time configuring tests to pull random data from a â€Å"datapool† so that each emulated user uses unique information. Performance Tester automatically identifies likely candidates for datapool access and makes it possible to associate these fields with data sources you provide. 1. Select the Enter Adventure Package Details page node in the test contents. Notice in the Test Data area the start_month, start_year, and start_day parameters. Note that the graph on the Resources tab will be blank: this is expected. When the test has finished (note the progress bar on the Overall tab), go back to the Page Performance tab. You should notice that the Checkout page is considerably slower than any of the other pages. 4. Section 7. Analyze the root cause At this point, you have successfully used Performance Tester to uncover a performance problem in your application. The next question you will ask yourself is, â€Å"What is causing the problem? † To get to the bottom of this question you will use Performance Tester’s Root Cause Analysis facilities. In this section, you will re-run your test with additional data collection tools engaged. The additional information will help you determine if you are facing a hardware or software issue and drill down to the root cause of the performance bottleneck. Revisit some of the additional execution controls on the schedule: start by selecting the top node of the Schedule Contents again. Resource monitoring enables Performance Tester to log any system parameter from Windows perfmon, Unix or Linux rstatd, or Tivoli Monitoring. Note that in this trial environment, the Web server, application server, and Performance Tester system are all running on a single machine: localhost. This is, of course, not a realistic situation. In a true performance testing environment, you can define any machines that might be part of your application or test system. Select the Resources tab. After a few seconds, you can see the extensive list of counters available. To keep it simple, deselect all counters except Memory Pages/sec and Processor % Processor Time. The Resources tab on the Performance Report . The data you see here, although accurate, is not really representative of a typical load test. In this trial environment, the Performance Tester load generation, Web server, application server, and database server are all running on a single machine. You would normally track resources on each tier of your application. In addition, the load test you just ran was of very short duration. Normally, performance tests will be significantly longer, allowing the systems to reach a steady state. Nonetheless, the trial system gives you a sense of how easy it is to track resource utilization during a performance test. If you have a concern, you can now go to the Response vs. Time Detail tab, right-click on the graph, and click Add/Remove Performance Counters to overlay resource counters onto page response data. Since it doesn’t appear that the performance issue with Adventure Builder is hardware related, use the response-time breakdown data to find out if it is software related. Examine the response-time breakdown data 1. 2. Select the Page Performance tab of the Performance Report. Drill down into what went on behind the scenes for the Checkout page. Right-click on the bar in the graph for the Checkout page and choose Display Response Time Breakdown Statistics†¦. Select the /ab/checkout. do URL from the Selection Wizard and click Finish. The Response Time Breakdown Statistics view lists methods called on the server tiers of the Adventure Builder application. There are various ways to examine this information. In a very short time you were able to construct a test suite by recording a performance test for a Web application, customizing the test to randomize data upon playback, and using that test as part of a realistic performance test schedule. You executed that schedule on a small scale and used the near real-time reports to identify a slow page. Once you had identified the page, you gathered resource utilization data and response-time breakdown statistics, which you then used to investigate possible hardware and software causes. George Orwell was the first to put these concerns onto paper in his book 1984. In his book, Orwell talked about the invasion of government into our lives, the effect that it would have on our freedom and its reprocusions in everyday life. He focused on the fact that we may someday have to live with cameras around evey courner and Big Brother constantly looking over our shoulder. Although compared to Orwells prediction of life in 1984 we are realativly free of Big Brother, everyday I see more invasion of privacy, from security cameras at school to placing tracking devices in cars. An excelent example is an incedent I was recently involved with at Boulder High School (BHS). It was about a week befor the seniors would graduate and the pranks began to fly. Freshman after freshman was creeked by various seniors. This ritual has usally been accepted by the studentbody and facualty as just kids being kids but not this year. This year the assitant princeapals had taken it upon themselves to put an end the hazings with the 13 brand new security system and a sophisticated control center in the office (costs exceeded $200,000). Upperclassman were being sent to the office in droves and some of the hazing slowed down. But then one day I was down by the creek coming back from lunch with friends, and we saw three freshman being creeked. This has become a common scene at BHS so we simply gave a passing glance and continued on to class. By the end of the day we all wound up in the office. We spent over two hours there during which our backpacks and lockers were searched. Appa rently the had gotten a glimpse of us (and not the actual perpotrators) on the cameras. It took the last two weeks of school of us comming down to the office on a daily basses, missing hours of class, and calls from parents befor the facualty would talk to us. We eventually convinced them not to suspend us but it was only by going to the princeapal that the school admitted its mistake and out records were expunged. Many people and organizations have voiced opnions on the issue of security cameras and there have been many proposed solutions. The only possible solution I see working is to remove the cameras from place owned by the government. I fell that this not only address the problem of privacy but also that of the cost. The procreation process is quite pleasurable and therefore the process is widespread and has many different variations tailored to people’s desires. Different people have different preferences. This paper focuses on a taboo that has been around for centuries and is still considered grotesque and detestable now as it was long ago. Necrophilia is a sexual attraction to corpses (Necrophilia). The prefix necro is Greek, meaning â€Å"death†, and philia is Greek for â€Å"love†. I choose this topic because of the interesting debate in class about Faulkers short story, â€Å" A Rose for Miss Emily†, that insinuated that she was a necrophilic. ?Necrophilia isn’t a new phenomenon. It has been going on since ancient times, with humans and even in the animal kingdom. Important Egyptian men kept their wives 7 days after death in attempt to prevent the embalmers from sexually defiling their corpses. Guards would guard tombs to protect corpses from grave robbers who would either rob the corpses or perform sexual acts with it. (Robinson 778). Legend has it that king Herod of Judea had sex with wife for 7 years after she died ( insert citiation). Many species of birds such as the mallard duck, jay bird and swallow who were observed engaging in necrophilia with the mallard duck having a homosexual necrophilic relations. ( insert citation) ? There are three different types of necrophilia. The pseudo, regular and homicidal necrophile, vary in degrees but all involve sexual arousal and gratification from a dead body. The pseudo-necrophile only fantasizes about corpses. They can role-play with a live woman who is told to be completely still and even powdered up. These lesser scale versions of necrophiles also get gratified by just viewing a dead body(insert cition). ?Another type of necrophilia is called regular necrophilia where the only way gratification is received is through actual intercourse with a dead body. Since the way to sexual pleasure is through dead bodies, these people find jobs or careers that place them around the dead. Necrophiles most common access to the dead is through their jobs. These occupations are ambulance driver, morticians, and cemetery employees. ( insert cite) ? The last type and more horrific type of necrophile are called a necro-sadistic offender which is a homicidal necrophile. These necrophiles are felons not because of their sexual acts, but because they obtain their corpses by actually killing people unlike the regular necrophiles who obtain them through their jobs. Some of these necrophiles engage in mutilation the body. The mutilation also stimulates and satisfies the offender sexually. Experts claim this type of necrophilic activity is the rarest. ( cite source) One would think that for someone to actually kill someone, just to carry out a sexual fantasy, would mean that they are mentally deranged. ?Some necrophiles have been diagnosed with personality disorder but surprisingly a high percentage of them have above average intelligence and normal personalities. According to ______ , â€Å"Necrophiles have been identified as insensitive to others, possessing a deep hatred for women, and experience an overwhelming fear of rejection. In the latter instance, the perpetrator compensates for feelings of inadequacy inasmuch as the actual sexual activity with a corpse provides the necrophile with sense of having the victim in complete submission to his authority and will. Rejection does not happen, failure does not occur, and humiliation is impossible.
